Fatal massive haemoptysis after embolectomy for chronic pulmonary embolism.

چکیده
Since the first report of successful "late" embolectomy for recurrent pulmonary embolism,' a number of patients have been described with significant pulmonary hypertension where clinical improvement associated with lowering of pulmonary artery pressure has followed removal of extensive, well-organised thrombo-embolic material from major pulmonary vessels.2 The following case history illustrates an unfavourable and highly unusual outcome in a patient with chronic pulmonary embolism complicated by severe, sustained pulmonary 'hypertension. Embolectomy was followed, five months later, by sudden death with massive haemoptysis from a fistula be-|X tween a pulmonary artery and main bronchus, a complication not previously reported. l _ _

Chronic Thromboembolic Pulmonary Hypertension after Pulmonary Embolism, Thrombolysis, Catheter Fragmentation, and Embolectomy
A 21-year-old male patient with massive acute pulmonary embolism was treated by thrombolysis, interventional thrombus fragmentation, and surgical pulmonary embolectomy. Within the following 2 years, the patient developed progressive dyspnea at exertion. Chronic thromboembolic pulmonary hypertension was diagnosed by right-heart catheter, VQ scan, magnetic resonance, and conventional pulmonary an...
متن کاملAcute pulmonary embolism: a current surgical approach.
Acute massive pulmonary embolism has a high mortality rate despite advances in diagnosis and therapy. Thrombolysis and catheter embolectomy have recently shown various degrees of failure and adverse effect. Surgical embolectomy has now been liberalised for haemodynamic stable patients with right ventricular dysfunction. We report our surgical experience in the last ten years including massive a...
متن کاملA Report on Emergent Pulmonary Embolectomy
Introduction: Pulmonary embolism is one of the leading causes of mortality in patients. The mortality rate of this disease can be significantly reduced with appropriate treatment. Surgical intervention can be highly effective for the treatment of acute massive pulmonary embolism. This article presents a report on the experience of acute pulmonary embolectomy. Materials and Methods: Demographic d...
متن کاملMassive pulmonary embolism: the place for embolectomy
Untreated massive pulmonary embolism is associated with a high mortality. Pulmonary embolectomy has been largely superceded by thrombolytic therapy, but there are cases in which pulmonary embolectomy remains the treatment of choice. We present three case reports and discuss the merits of the various treatments available for massive pulmonary embolism. The primary treatment of massive pulmonary ...
متن کاملPost Splenectomy Fatal Pulmonary Embolism in a Patient with Moderate Hemophilia A
Hemophilia A is a bleeding disorder caused by defective production of factor VIII. The main concern associated with the disease is bleeding, especially after trauma and surgeries. Factor VIII replacement therapy is associated with substantial decrease of bleeding events during surgery. However, there have been a number of reports of thromboemblic events in this situ-ation. The present report de...
